Normal prostate size and weight: grams, millilitres and when it matters
Your ultrasound report gives a figure in grams or cc and no explanation. Here is what the number means, what is normal for your age, and the reason your urologist cares about it far less than you do.

One thing first, because it saves reading the rest in a panic. Grams and cubic centimetres are the same number here. Prostate tissue has almost exactly the density of water, so a gland reported as 32 cc weighs about 32 grams. If your report says one and your doctor says the other, they are not disagreeing.
Normal prostate size and weight by age
From ultrasound series. These are averages: a man of sixty with a 25 g gland is not unusual and neither is one with 45 g.

Dimensions on a report are usually about 4 cm across, 3 cm front to back and 2 cm top to bottom in a normal gland.
What size of prostate is dangerous
Strictly, none of them. Size is not a danger in itself — benign growth does not become cancer however large it gets, and a large gland is not a sign of one.
What size does predict is trouble, and the thresholds urologists work to are these. Above 30 g the gland is called enlarged. Above 40 g, the DHT-blocking drugs start to make sense, because below that they have little to work with. Above 80 g, standard TURP surgery becomes technically harder and surgeons look at other approaches.
The danger is not the number, it is retention — being unable to pass urine at all. That is a same-day hospital problem and it needs a catheter, not a discussion.
Why a 30 g prostate can be worse than a 70 g one
Benign growth starts in the transition zone, the small collar of tissue wrapped directly around the urethra. Growth inward squeezes the tube; growth outward pushes into tissue that has room to give. Two men with identical 40 g glands can have completely different symptoms depending on which direction the tissue went. This is why your urologist asks you seven questions about your symptoms and glances at the size only afterwards.
How prostate size is measured on ultrasound

Transabdominal ultrasound
Through the belly with a full bladder. Non-invasive and widely available at any private lab in Pakistan. Slightly less accurate than the alternative.

Transrectal ultrasound (TRUS)
A small probe in the rectum. More accurate, mildly uncomfortable, and the standard when the number needs to be right — before surgery, for instance.

Digital examination
A gloved finger. Gives a rough estimate and, more usefully, tells the doctor whether the surface is smooth and even or hard and irregular. Free, immediate, and the one men avoid for years.
Can prostate size be reduced?
Finasteride and dutasteride reduce prostate volume by roughly a fifth to a quarter over six to twelve months. That much is real and measurable. What is also real is PREDICT — 1,095 men, 52 weeks — where finasteride alone separated from placebo on neither symptom score nor flow rate, while the alpha blocker did. A smaller gland did not mean a better year.

What is the normal size of the prostate in grams?
About 20 grams in a healthy adult man, which is roughly a walnut. Because prostate tissue is about the density of water, 20 grams and 20 cc are the same thing.
Is a 40 gram prostate enlarged?
Yes — anything over 30 g counts as enlarged. Forty grams is common at sixty and may cause no symptoms at all. Whether it needs treating is decided by how you feel, not by the figure.
What size of prostate is dangerous?
No size is dangerous in itself. Above 80 g, standard TURP surgery becomes technically harder. The genuine emergency is acute retention — not passing urine at all — and that can happen at almost any size.
Does a bigger prostate mean worse symptoms?
Surprisingly, no. The correlation between volume and symptom score is weak, because what matters is the direction the tissue grew, not how much of it there is.
Can I measure it at home?
